What Happens During Rhinoplasty Surgery?

The open rhinoplasty technique allows for all cartilage and bone components from the top of the nose down to the tip to be clearly visualized and accurately shaped. This includes an abundance of detailed procedures to correct everything from a large dorsal hump to oversized or flaring nostrils. Reduction of a nasal hump and refinement of the nasal bridge are common goals during rhinoplasty.

Often, septoplasty is performed to harvest septal cartilage, which is used during rhinoplasty. Septal cartilage is a very useful cartilage grafting material during rhinoplasty.
Open and closed rhinoplasty describes how we access the structures of the nose. An open approach uses a columellar incision as well as incisions inside the nose. A closed approach uses only incisions inside the nose.
General anesthesia is typically used during rhinoplasty. Modern general anesthesia is very safe. Use of general anesthesia helps protect your airway during surgery, which is very important.
When rebuilding the structure of the nose, either cartilage or bone is used to help contour and shape the nose. These grafts can be taken from the nose itself, the patient’s own rib or ear, or through cadaver cartilage.

External sutures are typically removed between three to five days after one of your in-office visits.
Splints are removed between five to seven days after your procedure. Your new nasal contour will be immediately visible; however, swelling may persist for many months.
No, we do not use nasal packing as it was classically used in the past. We use splints inside the nose that are typically in place for a week after surgery. Splints do not fully obstruct the nose but can make breathing more difficult during that first week.
You will have a cast on your nose, splints possibly on the inside of the nose, and perhaps some sutures at the columella and inside your nostrils. You will have swelling, and some level of bruising can be expected. Nasal congestion is common immediately after rhinoplasty surgery.
We ask that patients take it easy for the first week. Patients have a cast on the nose, and potentially splints on the inside of the nose for the first week. These are typically removed one week post-operatively. Ease back into normal exercise after two weeks postoperatively.
In general, after 1 week, the cast and sutures are removed. Many patients are comfortable returning to work at that time, depending on the nature of the work. Some prefer to take a bit more time before returning to work.
Walking is recommended after surgery. Ease back into normal exercise after two weeks postoperatively. The risk of bleeding in the nose is most present after surgery for the first 1-2 weeks. Keeping blood pressure down and limiting physical exertion is advised to avoid any postoperative bleeding.
Our pre-op department requires that patients quit smoking tobacco and nicotine products for weeks before and after surgery. Smoking causes prolonged healing and potentially worsened scarring.
Revision rhinoplasty is more challenging than primary rhinoplasty for a variety of factors. There is additional scarring to contend with during surgery and the postoperative period. Scar tissue from previous procedures can make revision cases more complex. Often, there is deformity or a lack of existing cartilages in the nose, thus requiring additional cartilage sources for reconstruction. There is an increased risk of asymmetries as well as tissue loss with revision cases.
It is normal to have some discomfort after rhinoplasty. You often have splints and casts in place, as well as sutures, which can cause discomfort during the initial healing phase.
